First mistake is 5 defending 6. Brooks makes the error by going in hard on Reynolds who draws, Luciano has Reynolds covered, Mbye comes in on Jimmy Jet (the right play) and Hoffman fails to follow him in, meaning Walker is under no pressure.

It was a penalty 81.25% of the time. 18.75% of the time is when the Tigers, Dog and Titans are defending.

4 on 5, Tigers not sliding, but Mbye correctly guesses Latrell is getting the ball and goes in hard, gets to him, Hoffman follows in. But Mbye misses Latrell, who floats a pass over Hoffman to Gagai.
It's Mbye's mistake but he made the right play, he just didn't make the tackle. 1-1 Latrell is always going to be an issue. Hoffman turns like a bus and because Mbye has missed Latrell, Brooks is forced to cover and can't slide out.

4 Tigers on 5 Rabbits again, Mbye thinks Latrell is getting the ball and rushes out. He could legitimately have held his line, which is probably the best play because Su'A has already gone in-field and Brooks has not taken that bait, Brooks could have held Latrell and Mbye stayed on Jimmy Jet.
Problem is when Mbye goes in, Hoffman follows him in without watching the attack, so he runs in on a gap. The out-ball hits Roberts, so Mbye's play on Latrell has no impact and Hoffman is covering nobody. Hoffman needed to hit Roberts once Mbye goes in.

That is 100% a defensive decision and Roberts does the correct thing by running to inside shoulder. Mbye makes literally no attempt to slide or move out.

No idea what's going on here, why so compressed. I can see why Mbye moves in, because the player in the first frame, Roberts, scores untouched if the ball hits him. Luciano and Brooks have not made any lateral move at all and we are on our heels on the try line.
Hoffman doesn't help the situation by making a really late decision to attack the 2nd-man, which is Reynolds. He probably should have moved up and held his line longer to see if Mbye could recover in cover.

So I put the first try mostly on Brooks, second mostly on Mbye, 3rd mostly on Hoffman, 4th mostly on Brooks.
Unfortunately in Try 2 and Try 4, Hoffman makes a secondary bad decision that essentially reinforces the primary mistake. This is evidenced by the fact that Gagai strolls over untouched, which should never be the case, there should always be some kind of pressuring cover even if you have been exposed by a wrap play.

I think in Hoffman's case it was a situation of follow the leader...he would of been quite nverous and having the captain and a experienced first grader inside him he obviously would be inclined to follow mbyes moves

Thanks for the analysis but I disagree with the summary.
The first one is a penalty to us, if, Mbye falls to the floor.
However
Mbye comes up with the line holds his position as the ball is being moved out the back.
Turns left and is hit by a hard running Roberts who starts on his outside shoulder and finishes inside.
Penalty all day, everyday to a more fancied team.
