I think a Koroisau/Cook combination at hooker is by far the best option for NSW. It would provide 80 minutes of constant threat out of dummy half. Cook also has utility value as a replacement fullback or half (and Koroisau is also a backup half).
80 minutes of speed out of dummy half is going to be more valuable than having a 4th forward on the bench when a lot of the forwards being selected are capable of big minutes anyway.

The way Teddy passed the ball last night when closish to his own line instead of taking off with his speed and fancy footwork - he threw his chances away. I think the crowd was disappointed as well, as well as being dangerous throwing the ball long distance to other player close to line.

Moylan was exposed at fullback for his lack of genuine pace. I can't see how he can continue in that position.
