Dominant Referee - controls game in all aspects.
Second Referee - controls ruck, and only passes advice to Dominant Referee when asked to do so.
Touch judges - forward passes and 10mtrs, assist Referee when asked to do so. Offsides indicated by raising of white flag NFL style.
Video Referees - should be 3 of them. They sit separate, and can't communicate with each other. They all check replays before making a decision. None of them to know what the other is ruling. Majority rule applies on a decision, eg 3-0 or 2-1.
Let everyone know their duties and be accountable.
